BO4 assessment and evidence checklist

• Confirm the entity is within the regulations and record its registration date.

  Evidence: Current company extract and scope note, including any relevant listed-company exclusion.

  Owner: Company secretary and adviser

• Reconcile registered ownership and voting rights.

  Evidence: Register of members, constitutional documents, share movements and current ownership diagram.

  Owner: Company secretary

• Identify fiduciary and nominee arrangements.

  Evidence: Declarations and underlying agreements showing the capacity in which each holder acts.

  Owner: Shareholders and adviser

• Investigate ultimate control beyond the register.

  Evidence: Shareholder agreements, appointment rights, enquiry records and reasoned assessment.

  Owner: Director and adviser

• Address any senior-managing-official fallback correctly.

  Evidence: Record of exhausted enquiries, difficulties and basis for the identified official or officials where appropriate.

  Owner: Director and appropriate professional

• Verify identifying particulars and effective dates.

  Evidence: Identity records, place of birth, address, nationality, nominee details and dates of ownership changes.

  Owner: Company secretary

• Record whether all four simplified conditions are met.

  Evidence: Dated assessment, relevant provision, unresolved issues and any MBR clarification needed.

  Owner: Director or company secretary

• Complete the applicable submission and retain evidence.

  Evidence: Current form, officer's signature, filed version, acknowledgement and follow-up record.

  Owner: Authorised filer and signatory

• Update the ongoing corporate calendar.

  Evidence: Separate entries for reportable changes, annual BO confirmation where applicable and the ordinary annual return.
